You'll also need to confirm that you have reviewed the guide to a Risk Management Program at your martial arts gym, and be prepared to answer "Yes" on the next page to move forward and apply (page 3).

“Boxing” = Regulated sport of boxing (not general striking/sparring)
“Law Enforcement Training” = Programs designed exclusively for LEOs
“Pro Fighters” = Any athletes at your gym paid to compete professionally
MMA is OK, as long as you don’t have a boxing ring or MMA cage
